Home Medicines Review (Item 900)

We work in conjunction with an independent Clinical Pharmacist who specialises in Geriatrics. A Home Medicines Review is a service that allows a thorough check of all the medications your patient is taking by a pharmacist in your patient’s home.

We identify eligible patients while performing the over 75 Health Assessment and then arrange an ‘HMR Referral’. The aim is to achieve safe, effective and appropriate use of medications. Healthcare professionals can identify:

• How the patient is managing and complying with the medication regimen

• Address problems that come up swiftly and easily.

• Remove medications that are out of date or ceased.

We provide education on storage of medication, and education on use of non-prescription medication.

Home visits can detect problems not easily detected in a GP’s surgery.
