We develop this plan over a minimum of three hours, liaising with all providers. Each assessment consists of:

• An extensive medical and social history;

• A full health examination and discussion- inclusive of a mental status, mood and nutrition assessment;

• A home safety assessment, medication review and service requirements.

This detailed plan also includes all relevant test results and investigations and incorporates all care provider roles in your patient’s management and care. Our nurses provide ongoing monthly support for patients, enabling your patient to experience better health and a reduced risk of hospital admissions. Your patients become better educated and empowered to self- manage their conditions whilst remaining at home.

You and your practice receive recognition and compensation, including non face-to-face time spent in providing comprehensive care to eligible patients,  as well as helping to improve the quality of care for your patients.
